We accept most insurance plans, including no fault and workers compensation. Although, some insurance plans may have a co-payment or co-insurance. It is best to call the office to confirm prior to your appointment so you do not get any unexpected bills.
Some insurance plans require a referral to be seen by a specialist. Please double check with your insurance company to see if you require a referral prior to your appointment. If so, please make sure to send us your referral prior to your appointment.
Each of our offices has street parking. Our Jamaica location also has a parking garage off of 89th Avenue and the Van Wyck Expressway. Please make sure to bring your parking voucher and give it to the front desk when you come in for your appointment.
Yes, you can ask any of our staff whatever questions you have. Please let them know what kind of form you have so they can advise you on any questions you may have. Please note, all forms are completed within 7 to 10 business days from the day we receive them.
